She will be in charge of Inter Milan from 2021 to 2024, having previously led Juventus to four consecutive Serie A titles.
The 54-year-old also served as head coach of the Italy U-17 team.
Skinner was sacked by West Ham on Thursday after winning just one of 11 Women’s Super League games this season.
“I am delighted to join West Ham United. This is a club with a long history and strong values that align with my own,” Guarino said.
“I’m very excited about the program here. There’s a lot of talent and experience on the team and I want to support the girls’ development as individuals and as a collective.”

West Ham made a bold move by appointing Guarino, who was relatively unknown in England.
One of her top priorities is getting through the January transfer window as the Hammers look to move higher up the WSL table. They are currently second from bottom, two points above Liverpool.
“I want to build an organized, attacking team and try to recognize and exploit spaces. The mentality of the team – unity and hard work – is the most important,” she added.
“I look forward to the next chapter in the best league in the world.”
